Disabled Parking Placard Applications
The online fillable application can be found HERE. Temporary placards must be obtained from the Secretary of State’s DMV office in Elgin. Permanent placards must be obtained from the Secretary of State’s office in Springfield. The placard will be mailed directly to the applicant’s home. These forms are available online, or from your doctor’s office.
